What is ChoiceGround?

People have wishes about their future care. What they want. What they do not want. Who speaks for them if they cannot speak for themselves. These wishes are often expressed — but rarely recorded in a way that the right people can find and act on at the right moment.

ChoiceGround records a person's advance care planning decisions in a structured, witnessed State — confirmed by their GP, held in their network record, and reviewed whenever their situation changes.

Why this record matters — stated plainly
Without a Choice record
Wishes are expressed but not recorded. The ambulance crew does not know. The out of hours doctor does not know. A crisis happens and the wrong decision is made — not out of malice, but because no one could find what the person actually wanted.
With a Choice State
Wishes are recorded, witnessed, and held in the network record. The right people can find them. The DNACPR travels with the person. The person who speaks for them is named. The record is reviewed when circumstances change.
