What is FarewellGround?

At the end of a person's life, the coordination failure that has always mattered most becomes most visible. The wrong clinician arrives. The DNACPR cannot be found. The person dies in hospital when they wanted to die at home. The network did not hold.

FarewellGround opens when palliative care begins. It carries forward the person's expressed wishes. It names every open loop and holds them open until each one is confirmed complete. It does not close at the moment of death. It closes when the network has done what it promised.
