What is MedRecGround?

At admission, the GP medicines record looks complete. It is often wrong. Medicines stopped, doses changed, new prescriptions added — none of it reflected in the SCR by the time the patient arrives on the ward.

MedRecGround coordinates every person who knows what a patient is actually taking into a single verified medicines list — confirmed by a named clinician before the ward team acts on it.
