DETERIORATION MONITORING
What is WatchGround?

Some deterioration is not an emergency. It is a slow, consistent trend — blood results declining across months, function reducing across years. The danger is not the single result. It is the pattern that no single clinician is tracking, because no one holds the whole picture.

WatchGround opens a Watch record when a trajectory changes. It tightens the decay rate across all existing States, puts the whole network on notice, and sets a clear referral trigger so the next step is not missed.

Demo patient
Dorothy Hargreaves
Age 74. Kidney function declining over six months. Three results. A consistent downward trend that no one is formally watching.
The slow deterioration problem — stated plainly
Without a Watch record
Each blood result is seen in isolation. No one holds the trend. The referral threshold is missed. A medicine that was safe six months ago is no longer appropriate — but no one has connected the results to the record.
With a Watch State
The trend is named and locked. Existing States are flagged for re-verification. The referral trigger is set in advance. The decay rate tightens. The network knows what to watch for — before the threshold is crossed.
